Abstract

Non-communicable diseases are one of the main causes of death in the world with 36 million morbidity and 57 million deaths in the world population caused by non-communicable diseases. Data from the East Simeulue Health Center from 2017 to 2018, shows an increase in the percentage of non-communicable diseases, especially in hypertension (5.42%) in 2017 to (5.72%) in 2018 and Diabetes Mellitus (4.19%) in 2017 to (4 , 42%) in 2018. The research objective was to determine how to evaluate the implementation, constraints and solutions in the non-communicable disease program at the East Simeulue Health Center in 2019.
The research design used in this research is qualitative with a phenomenological approach. The research informants were 2 main informants, namely the Head of the Puskesmas and PTM Managers and 6 additional informants, namely the PTM Manager of the Health Office, a cadre and 4 patients. Data collection methods are primary, secondary and tertiary data. Data collection techniques are carried out by means of observation, in-depth interviews and documentation. Data analysis techniques in this study are reduction, display and conclusion or verification.
The results showed that the evaluation of the implementation of the PTM program at the East Simeulue Health Center was not optimal due to the lack of community empowerment, the role of PTM managers, PTM managers workload, too little allocation of funds, limited facilities and infrastructure, the role of cadres, the number of active cadres, cooperation with leaders. community and cross-sectoral support.
For the East Simeulue Puskesmas it is recommended to empower the community in changing their behavior through the role of cadres, religious leaders and community leaders, adding facilities and infrastructure through submitting BOK funds to the Health Office, collaborating with cross-sectoral and proposing training and adding human resources.
